Assessment, diagnosis and developing a care plan for younger, disabled adults or older adults over the age of 60 who may have substance abuse issues is now available through Evergreen Healthcare’s Behavioral Health Geriatric Regional Assessment Team. The new program started recently.
The Substance Abuse Services provider will visit individuals at their residence. The program includes assessment and diagnosis, individual and family therapy, case management, stabilization and treatment, consultation and care planning. The most common substances abused are alcohol and prescription drugs, particularly pain medications.
Referrals are accepted from any source, but should be discussed with the prospective client before the family or care giver calls in. Call 206-923-6300 or 1-800-548-0558 and ask for Intake 8 a.m. to 5 p.m. Monday through Friday.
Medicaid typically covers substance abuse services 100 percent and private insurance will cover all or part of service fees in most cases. There is a sliding fee scale available as well for those without insurance.
